To make the request, the person who desires the record should first make sure that he or she is eligible to claim the records because the records are not public records.   • Head over to the Department of Health
  • At the Vital Records Section, inform the clerk on duty of your intention to request for death records.
  • The clerk would ask you some questions about yourself and the records that you are interested in to determine if you are eligible to request for the same.
  • If you pass, the clerk would give you a request form that you must then accomplish.
  • Submit the accomplished request form and wait for the clerk to see if they have the records.
  • If they have the records, you would be asked to verify if those are the records that you are interested in.
  • Request for copies of the records.
  • Pay the required fee at the cashier office and return to claim your copies of the records from the clerk.
